The Registration Process for this event is a 2-step process. Before beginning the process, make sure you are eligible to attend this event:
Eligibility Requirements:
Only SEA/TEA/TG partners funded for HIV under the 801 Cooperative Agreement and their team members are invited to apply to attend Strengthening Our Practice: Sustaining Our Efforts. Up to 20 teams can attend the event. To apply, teams must complete participant expectation agreements and meet the team composition requirements.

CDC Funded HIV Coordinator is the Team Lead – The Team Lead will serve as the primary contact on all communications related to Strengthening Our Practice: Sustaining Our Efforts.
Gather Information - The Team Lead gathers the name, title, and e-mail for each team member.
Submit Team Application for Approval - Submit the Team Application Form including participant expectation agreements for approval by the date specified for your session (SF) of the event.
Receive Approval – The Team Lead will be notified regarding the outcome of your application. If approved, all team members will receive an invitation to register, instructions for registration, team registration code, and a link to the Event Registration Page.
Step 2: Individual Team Members Register for Event
Register - Once team application is approved, all team members will receive email confirmation. Contained in this email will be a link to the Event Registration page. Once you receive your Team Code and registration links, all team members register for this event (STEP 2) by the date specified for your session (SF) of the event.
Proceed with Travel Arrangements - Once registered for the event, individual team members can then proceed with travel arrangements outlined on the Event Information page. Participants registered for San Francisco are asked to complete all hotel and air travel arrangements no later than the date specified for your session (SF) of the event.
Team Composition Requirements
Eligible funded partners, will submit an online TEAM APPLICATION FORM identifying a team of three participants including team lead for approval through the application process, for a maximum of 20 teams per event. Team participation is required. Team composition includes a total of three people:
